The following agencies have been funded for services to meet Special Health Care Needs in Fiscal Year 2012 (October 1, 2011 through September 30, 2012).  Services may be delivered through more than one service location and/or funding Agreement.  

Unless specified otherwise, all services are for youth ages 0-17 (or through age 21 if enrolled in an academically accredited program leading to a high school diploma or its equivalent).

Behavior Modification, Children's Respite (in-home and out-of-home) and other supportive services with chronic physical, developmental, behavioral conditions, who have mild to severe maladaptive behaviors
